A complete application consists of the 2010 Scholarship Application Form, the documents detailed below, and one interview. All documents (except the Affiliate President's report) must be postmarked by March 31, 2010.
[ ] One completed 2010 Scholarship Application Form is required--online or in print. (See FAQ #11-12)
You may enter N/A to any questions that are not applicable. For the online form, we recommend you copy and paste your carefully written essay from a document using word-processing software. Pressing the Submit button at the end of the online form sends your form to our office. After pressing Submit, you cannot return to this completed form; therefore, first save a copy if you wish one for yourself. You will receive a Confirmation Page to confirm that the form arrived in our database. Please contact the chairman if you do not receive confirmation, you have changes to your application, or you prefer we send you a print application.
[ ] Two Letters of Recommendation are required. (See FAQ #15)
The best letters are from worthy individuals who can describe your academic ability, leadership skills, and/or community involvement. They may mail or e-mail their letters to the chairman.
[ ] Transcripts are required. (See FAQ #14)
Provide us with a transcript from the institution you are currently attending and transcripts from all other postsecondary institutions you’ve attended. If you have attended less than one year of higher education, provide your high school transcript. We readily accept transcripts mailed or e-mailed directly from institutions. The chairman may or may not accept transmittals from a student.
[ ] High school seniors only--Reports on College Entrance Examinations are required. (See FAQ #14) Include copies of your reports for the ACT, SAT, or other such standardized tests.
[ ] Proof of Legal Blindness is required. (See FAQ #3 through #6)
Provide a statement from a qualified professional that certifies you meet the federal definition of “legally blind.” Certifying Authority: The NFB form is preferred; alternately you may submit a signed statement on business letterhead. Either one must be completed by a trained certifying authority, such as an optician, optometrist, ophthalmologist, physician; itinerant teacher of the blind, rehabilitation counselor, or other professional in work with the blind; or the president of a local chapter or state affiliate of the National Federation of the Blind.
[ ] One Affiliate President’s interview is required.
All applicants are required to contact the nearest NFB affiliate president before March 31 to set up a personal interview. A president or president's designee completes the interview, usually by telephone, then sends a report to the scholarship chairman. The Federation has affiliates in every state, the District of Columbia, and Puerto Rico. If you live in one state and attend school in another, you may contact either affiliate president. (Ask whether there are affiliate scholarships.) For contact information, go online to www.nfb.org (Topic Index, Affiliates, choose state), or contact the NFB Scholarship Program chairman. Note that some presidents may choose to conduct their interviews after the student applicant deadline of March 31, therefore do not wait for the interview before forwarding your documents to the chairman.
